no one can sign up

As far as I can tell it is all setup right, but when you go to the site and put your username, email, and password, then click signup, it takes you to a blank page on my web site... see below. http://www.loveconnectionlive.com/dating/member/join.php? And there is never any email verification sent out to the new user. What is wrong?
